SPECIFICATIONS, POLICIES, CONSENT WAIVER FROM LIABILITY & MEDIA RELEASE
I hereby attest that I do not suffer from any heart, lung, or other medical condition or disease that might in any way hinder or prevent me or my child from participating in the music lesson/special event activities available at G-Clef Music Academy's 2024 Ukulele Workshop (Activity). I assert that the child that I am registering for this workshop, through this application, is between the ages of 6 and 13 at the time of this Workshop/Activity. I agree that I and/or my child participate at my own risk. In consideration of my signing this agreement, I hereby for myself, my heirs, and administrators assume any and all risks which might be associated with this event. I waive and release any and all rights and claims for damages which I may have against the organizers and any others connected with this event, their representatives, successors and assigns for any and all injuries or damages of any kind whatsoever suffered by me or my child as a result of taking part in the event and any related activities.
I further acknowledge that the novel coronavirus known as COVID-19 has been declared as a worldwide pandemic and is believed to be contagious and spread by person-to-person contact, including the Virgin Islands. I further acknowledge that when applicable, the federal, state, and local agencies recommend social distancing and other measures to prevent the spread of COVID-19.
In an effort to ensure the safety and wellness of our community, I will comply with required handwashing upon entry into the G-Clef Music Academy facility, as well as the use of hand sanitizers when required. I and my child participant will, when necessary and applicable, wear nose/mouth masks and practice social distancing.
In addition, by selecting "AGREE" below, I agree that I will:
-Not permit my child, or myself, to participate in the Activity, if my child, or I have a fever, until he/she has been without a fever for at least 72 hours. A fever is defined as a temperature over 100.4 F or 38.0 C.
-Make a visual inspection for signs of illness which could include: fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss or taste of smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea, flushed cheeks, rapid breathing or difficulty breathing (without recent physical activity) fatigue, or extreme fussiness. If we have exhibited any of these signs or symptoms, I will not permit my child(ren) to participate in the activity until he/she has been without signs or symptoms for at least 72 hours.
-Promptly pick up my child(ren), or arrange for pickup, if signs or symptoms of illness are present. I understand that children are to remain home until illness-free for at least 72 hours without the use of medicine.
--Allow G-Clef Music Academy to capture photographs, images and video footage of participating student and myself. G-Clef Music Academy uses photographs and video footage of students participating and Academy-related activities in its publications, promotional materials, brochures, and on internet-based promotional sites, like G-Clef Music Academy’s website, Facebook, YouTube, Instagram, and other social media.
The undersigned parent and natural guardian or legal guardian does hereby represent that she/he is in fact acting in such capacity and agrees to defend, save, hold harmless and identify each and all parties because of any defect or lack of such capacity to so act and release said parties on behalf of both the minor and parent or legal guardian.
We accept valid credit/debit cards. Any balance owed to G-Clef Music Academy MUST be paid prior to registering to 2024 Ukulele Workshop or any class to be scheduled. All scheduled workshops and music lessons with G-Clef Music Academy are pre-paid.
REFUND/DISCONTINUATION OF UKULELE WORKSHOP POLICY: There will be NO REFUNDS or reimbursement for any funds, and NO make up workshop sessions will be granted.
